Solanto, Ph.D.&lt;br /&gt;Director, Attention-Deficit/Hyperactivity Disorder (ADHD) Center&lt;br /&gt;The Mount Sinai Medical Center&lt;br /&gt;&lt;br /&gt;When Ethan brought home his first fourth grade report card, he knew his parents would not be pleased. In addition to grades that ranged between B and D, the teacher had commented, just as had his third grade teacher, &quot;If Ethan would just focus on his work and try harder he could do so much better!&quot; &lt;br /&gt;&lt;br /&gt;Lack of concentration and apparent lack of effort are hallmarks of the Inattentive subtype of ADHD. The Predominantly Inattentive type (IN) of ADHD, without hyperactivity or impulsivity, has been recognized as a disorder since 1994. Unlike children with the better-known &quot;Combined&quot; (CB) (inattentive, hyperactive and impulsive) form of the disorder, children with IN are not behaviorally disruptive and are not typically difficult to manage for parents or teachers. Instead children with IN may instead appear to be underactive, sluggish, or daydreamy. Their problems with attention are just as severe as those with CB-type ADHD, however. When they should be working on an arithmetic problem sheet, they may instead be found gazing into space. When called upon in class, they may have no idea what the discussion is about. Not absorbing new information or producing work as well as their classmates, children with IN may be mislabeled as &quot;slow learners&quot; or &quot;learning disabled,&quot; often on the basis of group-administered tests, which they were also not able to concentrate on. Even more often they just quietly underachieve, with no one fully aware of their true potential. &lt;br /&gt;&lt;br /&gt;In addition to their academic difficulties, children with IN often have very significant social problems. They may seem to their classmates to be &quot;tuned out,&quot; and not &quot;with it.&quot; Described sometimes as &quot;space cadets,&quot; studies show that children with IN are more likely than other children to be &quot;socially neglected&quot; — that is, to be ignored and friendless. &lt;br /&gt;&lt;br /&gt;One recent study suggests that the social problems of children with the IN and CB subtypes of ADHD may have different causes and different results. Children with the CB type tend to annoy other children with their provocative, intrusive, and sometimes aggressive behavior. As a result these children are more likely be socially rejected. When asked directly, however, these children with CB are able to state clearly and accurately how they should and should not behave with other children. In the heat of the moment, however, they are unable to exercise the behavioral self-control to follow through. Children with IN, on the other hand, were more likely to actually be lacking in knowledge of social skills, including how to go about such everyday social interactions as joining a group of children already at play, resolving a dispute, and initiating a friendship. In part, these difficulties may be due to a failure to pay attention to the social nuances, such as the nonverbal cues or &quot;body language,&quot; that are an important part of social communication. &lt;br /&gt;&lt;br /&gt;A Prevalent and Impairing Condition&lt;br /&gt;Survey studies suggest that the IN form of ADHD may be at least as common in schools as the better-known CB type. In these large-scale studies, hundreds of teachers were each asked to rate all the children in their classes on a checklist of behaviors reflecting the three core symptoms of ADHD: inattention, hyperactivity, and impulsivity. &lt;br /&gt;&lt;br /&gt;The survey data also showed an interesting distribution between the sexes. Whereas the CB type is about 4 times more common among boys than girls, the IN type is more evenly distributed between the sexes with a boy:girl ratio closer to 2:1. In fact, if a girl has ADHD, she is more likely to have the IN type. &lt;br /&gt;&lt;br /&gt;In the survey studies teachers were also asked to rate how &quot;impaired&quot; students were in the three most important areas of functioning in school: academic, social, and behavioral. The percentage of children rated by teachers as being &quot;impaired&quot; are shown below for the three currently recognized subtypes of ADHD: IN, CB, and Predominantly Hyperactive-Impulsive (PI). &lt;br /&gt;&lt;br /&gt;&amp;nbsp; IN CB HI &lt;br /&gt;Academic 76% 82% 23% &lt;br /&gt;Social 59% 82% 53% &lt;br /&gt;Behavioral 58% 90% 80% &lt;br /&gt;&lt;br /&gt;Source: Gaub, M. &amp; Carlson, C. (1997) Journal of Abnormal Child Psychology, 25, 103-111.&lt;br /&gt;&lt;br /&gt;These findings showed that a high percentage of children with IN were suffering academic difficulties—in fact, the percentage equaled that for the CB type. Sizeable percentages also had behavioral and social difficulties. &lt;br /&gt;&lt;br /&gt;Despite the serious difficulties that children with IN experience, relatively few are identified or referred to treatment. Children with IN account for only 25 percent, at most, of all children with ADHD who are seen at mental health centers. One likely reason is that, since they are quieter and far less disruptive than children with CB, they are less likely to create headaches for teachers or parents, and thus more likely to be overlooked. &lt;br /&gt;&lt;br /&gt;Another important finding emerging from the survey studies was that the IN type of ADHD may have a later age of onset than the CB or HI types ADHD. Although all subtypes must have an onset by age 7 in order to meet current DSM-IV criteria for the ADHD, these more recent studies suggest that many cases of IN type may actually not become apparent until age 9, and that impairment may not be significant until age 11. &lt;br /&gt;&lt;br /&gt;Assessment&lt;br /&gt;Like all good psychological assessments of children, the assessment for ADHD begins with a an interview with the parents or guardians concerning the details of the child&apos;s current difficulties, including time of onset, frequency, duration, and severity. Behavioral questionnaires, completed by parents and teachers, are also very helpful in determining the nature of the child&apos;s difficulties and their seriousness when compared to the behavior of other children of the same age and gender in the general population. During this interview, the parents will also be asked about the child&apos;s physical, mental, and emotional development from birth right up to the present. A good assessment will also inquire about the development of the family in order to identify any stressors or other problems that may have affected the child&apos;s functioning.&lt;br /&gt;&lt;br /&gt;The child is interviewed individually in order to get the &quot;child&apos;s-eye&quot; view of the challenges, satisfactions, and stressors in his or her life at school, home, and with peers. It also affords an opportunity to informally observe and assess the child&apos;s attention, language, self-control, self-confidence, and relational skills. &lt;br /&gt;&lt;br /&gt;When there is concern about a child&apos;s general learning ability, or specific &quot;information-processing&quot; skills (as is often the case in all types of ADHD), a set of tests may be administered to more precisely examine the child&apos;s intellectual functioning, as well as his/her current level of educational achievement in the major subject areas. Neuropsychological tests, which assess brain-based functions in the areas of memory, language, attention, and motor skill, may also help to identify underlying causes of the child&apos;s difficulties. Because inattention is the primary symptom of IN and because the tell-tale signs of impulsivity and hyperactivity are absent, a good clinician will take particular care to rule out other &quot;silent&quot; problems, such as anxiety and depression, which can also impair concentration and effort, before diagnosing IN. &lt;br /&gt;&lt;br /&gt;Treatment&lt;br /&gt;There has been little study as yet of treatments that are specifically effective for children with the IN type of ADHD. Clinical experience at Mount Sinai&apos;s ADHD Center, however, suggests that the stimulant medications that are effective in the treatment of the CB type of ADHD may also be effective for IN. Research is also underway at Mount Sinai, supported by a grant from the National Institutes of Health, to investigate the unique difficulties of children with IN in orienting and focusing, immediate and short-term memory, and in &quot;executive&quot; functions, such as self-stopping, organization, and planning.   <description>MASTER CLEANSE &lt;br /&gt;&lt;br /&gt;About&lt;br /&gt;Before you start the Master Cleanse, you should already be on a balanced diet, like the ones &quot;From Flab to Fab&quot; participants eat during their 12-week plan. In each phase of the Cleanse, there are certain food items that are eliminated. Drink plenty of water! &lt;br /&gt;&lt;br /&gt;Ingredients&lt;br /&gt;&lt;br /&gt;4 table spoons Epsom salts &lt;br /&gt;3/4 cup Olive Oil (light olive oil is easier to get down; use only the best cold-pressed virgin oil) &lt;br /&gt;1/4 cup lemon juice, squeezed from fresh lemon (do not use concentrate or store-bought juice - make your own!) &lt;br /&gt;1/2 cup grapefruit juice, squeezed from fresh grapefruit &lt;br /&gt;1/2 teaspoon of Vitamin C powder &lt;br /&gt;Jar with a non-metallic lid &lt;br /&gt;&lt;br /&gt;Preparation&lt;br /&gt;&lt;br /&gt;Mix 4 heaping teaspoons of Epsom salts in 3 cups of water &lt;br /&gt;Add 1/2 teaspoon of Vitamin C powder &lt;br /&gt;Divide the salt/water/Vitamin C mixture into 4 servings, 3/4 cup each. &lt;br /&gt;Mix 1/2 cup olive oil, 1/4 cup squeezed grapefruit juice and 1/4 cup squeezed lemon juice in the jar. Shake briskly, until well mixed (almost watery - only freshly squeezed juice will do this). &lt;br /&gt;&lt;br /&gt;Day 1&lt;br /&gt;&lt;br /&gt;Take the first serving (1-of-4) of the saltwater mixture &lt;br /&gt;2 hours later, drink the second serving (2-of-4) of the saltwater mixture &lt;br /&gt;1 hour, 45 minutes later, drink the olive oil/juice mixture &lt;br /&gt;&lt;br /&gt;Day 2&lt;br /&gt;&lt;br /&gt;Wake up early, and upon wakening, take the third serving (3-of-4) of the saltwater mixture &lt;br /&gt;2 hours later, take the fourth (4-of-4) serving of the saltwater mixture AND drink the second serving (2-of-2) of the olive oil/juice mixture. &lt;br /&gt;&lt;br /&gt;NOTE: The liver flush will make you need to visit the bathroom frequently. You should lie down between bathroom visits! &lt;br /&gt;&lt;br /&gt;COLON CLEANSE&lt;br /&gt;&lt;br /&gt;About&lt;br /&gt;&lt;br /&gt;This regime is followed for a period of three consecutive days &lt;br /&gt;The only activities that should be done on these days are walking or yoga &lt;br /&gt;Drink plenty of water on all three days (about 8oz every waking hour) &lt;br /&gt;&lt;br /&gt;Breakfast &lt;br /&gt;&lt;br /&gt;1 Large bowl of watermelon and/or strawberries &lt;br /&gt;No defined amount - you can eat as much as you&apos;d like, until you are full! &lt;br /&gt;&lt;br /&gt;After Breakfast&lt;br /&gt;&lt;br /&gt;3 Fiber capsules &lt;br /&gt;&lt;br /&gt;Mid morning&lt;br /&gt;&lt;br /&gt;1oz. wheatgrass juice &lt;br /&gt;&lt;br /&gt;Lunch &lt;br /&gt;&lt;br /&gt;&quot;Free&quot; vegetable salad + 1 unsaturated fat (for instance, 1/4 avocado) &lt;br /&gt;&quot;Free&quot; means all the non-starch vegetables (which contain mostly water and fiber). Juice, made of equal parts beetroot, celery and carrot &lt;br /&gt;Any ONE of those ingredients can be substituted fro cucumber or spinach, depending on your own personal preference. &lt;br /&gt;&lt;br /&gt;After dinner&lt;br /&gt;&lt;br /&gt;3 Fiber capsules &lt;br /&gt;&lt;br /&gt;CASTOR OIL PACKS&lt;br /&gt;&lt;br /&gt;About&lt;br /&gt;Castor oil used to be used internally as a laxative to induce vomiting and diarrhea. It is now recognized as something that is better used externally, due to its potential toxicity. &lt;br /&gt;&lt;br /&gt;The castor oil cleanse stimulates the liver, increases lymphatic circulation, improved digestion and promotes &quot;elimination.&quot; &lt;br /&gt;&lt;br /&gt;Ingredients&lt;br /&gt;&lt;br /&gt;3 layers cotton flannel, size large. These are often sold at health food stores under the name &quot;castor oil pack&quot; &lt;br /&gt;Castor oil &lt;br /&gt;Plastic wrap &lt;br /&gt;Hot water bottle &lt;br /&gt;Old sheets (to lie down on) &lt;br /&gt;Water &lt;br /&gt;Baking Soda &lt;br /&gt;&lt;br /&gt;Preparation&lt;br /&gt;&lt;br /&gt;Soak flannel in castor oil so it absorbs enough oil, but is not so saturated that it is dripping. Allow any excess to drip off. &lt;br /&gt;Place an old sheet on the floor &lt;br /&gt;Lay down on the sheet and place the soaked flannel directly over the left side of your abdomen &lt;br /&gt;Wrap the plastic wrap around your abdomen so it holds the castor oil pack tight to your skin (you might need to wrap it a few times). &lt;br /&gt;While laying flat on your back. Place the hot water bottle on top of the plastic wrap &lt;br /&gt;Allow to sit for 45 minutes &lt;br /&gt;Remove the pack and cleanse the abdomen area with a dilute solution of water and baking soda &lt;br /&gt;Repeat this procedure for three days &lt;br /&gt;You can reuse the same castor oil sheet 25 to 30 times. Store the pack in a covered container in the refrigerator &lt;br /&gt;&lt;br /&gt;Safety&lt;br /&gt;&lt;br /&gt;Do not ingest castor oil. It can be toxic &lt;br /&gt;Do not apply castor oil to broken skin &lt;br /&gt;Do not do the castor oil pack if you are menstruating, pregnant or breast feeding &lt;br /&gt;&lt;br /&gt;You should consult your own physician before trying any cleanse routine, or drastically altering your diet in any way. &lt;br /&gt;&amp;nbsp;</description>
